Session description: The panelists will assess the efficacy of tools that measure healthcare transition readiness among pediatric organ transplant recipients. It will compare the outcomes (graft rejection, appointment/treatment adherence, mortality) of adolescents and young adults who participate in healthcare transition readiness programs versus those who do not.
