Study the efficacy and safety of combining Telitacicept with low-dose corticosteroids in the treatment of IgA nephropathy (IgAN) with crescent formation.
Selecting 72 patients diagnosed with IgAN accompanied by crescent formation through renal biopsy from June 2022 to February 2025 at our hospital as the research subjects, they were divided into a control group (n = 29) and a combination group (n = 43) according to the treatment method. The control group received conventional dose of corticosteroids, while the combination group received Telitacicept combined with low-dose corticosteroids. Clinical efficacy, changes in related indicators before and after treatment (24-hour urinary protein quantification, urinary red blood cell count, serum creatinine, urea nitrogen, serum albumin), and incidence of adverse reactions (acne, moon face, skin striae and ecchymosis, gastrointestinal reactions, infections, liver damage) were compared between the two groups.
The total effective rate after treatment in the combined group (91.3%) was higher than that in the control group (72.6%), with a statistically significant difference (P<0.05). Six months after treatment, the levels of 24-hour urinary protein quantification [(0.66±0.45)g vs. (1.31±0.52)g], urinary red blood cell count [(42.13±22.36) cells/μL vs. (72.16±55.31) cells/μL], serum creatinine [(69.89±19.32)μmol/L vs. (89.31±18.35)μmol/L], urea nitrogen [(4.99±1.61)mmol/L vs. (6.90±1.82)mmol/L] were all lower in the combined group compared to the control group, and the level of serum albumin [(41.22±1.98)g/L vs. (39.69±2.91)g/L] was higher in the combined group, with all differences being statistically significant (P<0.05). The incidence of adverse reactions in the combined group was 42.1%, while it was 37.3% in the control group. Among them, the infection rate in the combined group was slightly higher than that in the control group, but there was no statistically significant difference between the groups (P>0.05).
The combination of Telitacicept with low-dose corticosteroids is significantly effective in treating IgAN with crescent formation, can effectively improve renal function and reduce proteinuria, and has fewer adverse reactions and high safety.
