Chronic kidney disease (CKD) represents an increasing global public health problem due to its high prevalence and its significant impact on patients’ quality of life and healthcare systems. Progression to end-stage disease often requires renal replacement therapy (RRT), posing a major clinical and epidemiological challenge. Identifying the underlying etiology and associated comorbidities is essential for developing prevention, early diagnosis, and treatment strategies. However, there are no epidemiological reports characterizing this population in the state of Nayarit, Mexico.

To describe the etiology of end-stage chronic kidney disease in patients undergoing renal replacement therapy in the state of Nayarit. All patients diagnosed with end-stage chronic kidney disease who received renal replacement therapy between May 2023 and July 2025 at “Dr. Aquiles Calles Ramírez” General Hospital (ISSSTE) were included.

A total of 248 patients were included. Their main characteristics are summarized in Table 1. The leading cause of end-stage CKD among patients receiving RRT was diabetes mellitus (47%), followed by undetermined etiology (24%), arterial hypertension (11%), obstructive nephropathy (8%), glomerulonephritis (8%), and polycystic kidney disease (2%).

 

Chronic kidney disease is influenced by multiple factors, including geographic and sociocultural conditions. Understanding the etiology of end-stage CKD in each region is essential. In our study, diabetic nephropathy was the most common cause, consistent with other reports in Mexico. However, a high proportion of cases with undetermined etiology was observed, highlighting the need to improve diagnostic approaches and epidemiological registry systems in the region.
