Kidney transplantation provides the best outcomes for patients with end-stage kidney disease. Many factors are influence the preoperative evaluation and selection of allograft donors, including the Human Leukocyte Antigens (HLA) which play a significant role. Recent studies shown that compatible Human Leukocyte Antigen (HLA) match has a positive effect on graft outcomes. However, the role of HLA matching in kidney allograft survival is still unclear. While HLA matching remains important for long-term outcomes, its impact on short term allograft function appears to be limited. This study examined the effect of HLA compatibility on short term kidney allograft survival, measured by serum creatinine and urine protein levels at three months after kidney transplantation.
A single centre retrospective cohort study was conducted on living donor kidney transplantation performed between January 2023 and June 2025 at tertiary referral hospital in Yogyakarta, Indonesia. Demographic and laboratory data were collected from medical records. The primary outcomes assessed were serum creatinine levels and urine protein levels at three months following kidney transplantation. Spearman’s correlation analysis was used to evaluate the relationship between HLA compatibility and and three-month renal allograft function.
Thirty-six patients were included in the analysis. Recipients mean age 39.39±12.80 years, and most of the recipients were male (80.6%), HLA match were 38.72 ± 20.69. At three months, mean serum creatinine were decreased significantly from 6.70±4.31 mg/dL to 1.39±0.47 mg/dL and negative urine protein levels were increase from 39.1% to 64.5%. Overall, more HLA compatible were associated with decreased urine protein and creatinin serum levels at three months after kidney transplantations but not statistically significant (p>0.005).
HLA match did not significantly correlate with early allograft function as measured by creatinine, or urine protein levels at three months after kidney transplantation. These results indicate that many factors may influence the early post-transplant kidney function and more comprehensive study to determine the correlation between HLA match and allograft function. Further studies with larger sample sizes and longer follow-up period is recommended for better evaluation of factors that affect graft function.
