To explore the improving effect of budesonide enteric-coated capsules on renal function and urine-related indicators in patients with IgA nephropathy, and to provide evidence-based basis for clinical treatment.
The clinical data of IgA nephropathy patients treated with budesonide enteric-coated capsules were analyzed retrospectively. Serum creatinine (Scr), estimated glomerular filtration rate (eGFR), urine albumin-to-creatinine ratio (UACR), 24-hour urinary protein (24h UPro) and urinary sediment red blood cell count (URBC) were collected at baseline and 1 to 6 months after treatment. Repeated measures analysis of variance (ANOVA) or Friedman M test was used to analyze the dynamic change trends of the indicators.
The baseline characteristics of the included patients were as follows: age ranged from 22 to 69 years (mean, 39.70 years); Scr ranged from 55.90 to 500.70 μmol/L (mean, 129.49 μmol/L); eGFR ranged from 12.56 to 126.10 mL/min/1.73m² (mean, 75.40 mL/min/1.73m²); UACR ranged from 44.3 to 2825.0 mg/g (mean, 888.80 mg/g); 24h UPro ranged from 0.17 to 4.64 mg/24h (mean, 1.50 mg/24h); URBC ranged from 2.1 to 421.3 /uL (mean, 83.87 /uL); and 81.13% of the patients were treated with renin-angiotensin-aldosterone system inhibitors (RASi) in combination. During the 6-month treatment period, the Scr level of patients remained stable (never exceeding 400 μmol/L), and eGFR was maintained at 80-100 mL/min/1.73m². UACR decreased from the baseline mean of 888.80 mg/g to approximately 500 mg/g; 24h UPro decreased from 1.50 mg/24h to near the lower limit of the normal range; and URBC decreased from 83.87 /uL to near the normal range (0-5 /uL).
Budesonide enteric-coated capsules can effectively maintain the stability of renal function in patients with IgA nephropathy, significantly improve urine indicators, and have good safety. It is suitable as an important option for clinical treatment.
