CKD is a progressive condition characterized by the gradual and irreversible deterioration of kidney function, commonly caused by diabetes, hypertension, glomerulonephritis, and other chronic renal injuries. Persistent damage leads to inflammation, oxidative stress, and fibrosis, impairing kidney function. Perirenal Adipose Tissue (PRAT) has been recently recognized as an important homeostatic organ regulating the cardiovascular (CV) system and kidney maintenance via paracrine and endocrine mechanisms, as well as a potential marker of metabolic dysregulation, with significant implications for CKD patients. Excess PRAT has been associated with heightened inflammation, insulin resistance, and an increased risk of CV events—all critical factors influencing CKD progression. Emerging evidence indicates that PRAT thickness correlates with worse renal outcomes and higher CV risk in CKD patients. Studies have shown that increased PRAT thickness is associated with reduced estimated glomerular filtration rate (eGFR), higher proteinuria levels, and an elevated risk of adverse CV events, including hypertension and atherosclerosis. However, there is a paucity of data examining these relationships specifically in patients classified as "very high risk" according to KDIGO guidelines. In order to address this lacuna LiveKidney is conducting a data-driven, retrospective analysis of PRAT thickness in CKD patients in order to examine correlations with clinical outcomes and evaluate the impact of treatments on PRAT thickness. Additionally, the study will compare PRAT measurements obtained via ultrasound and CT to determine measurement consistency and reliability

This retrospective cohort study will analyze data from imaging studies, laboratory assessments and clinical investigations stored in electronic medical files of CKD patients treated in the Shamir Medical Center, Be'er Ya'akov, Israel collected over the period of 2019-2024. The primary objective is to evaluate the correlations between PRAT size and CKD progress obtained via ultrasound, MRI and CT. The secondary objectives are (a) to evaluate the correlation between CKD comorbidities (diabetes, high blood pressure, obesity) and PRAT thickness; (b) to examine the correlation between PRAT size and demographic data (age, gender, weight, BMI); and (c) to assess the impact of different treatment modalities on PRAT size. Machine Learning (ML) approaches employing predictive modelling methods, pattern recognition as well as validation and model performance metrics will be utilized to predict disease progression, thereby enabling more accurate selection of patient populations for further clinical evaluation.
