Chronic Kidney Disease (CKD) affects more than 7 million Filipinos and it is associated with great morbidity and mortality and more importantly, with diminished quality of life due to the overall burden of the disease. Patients with chronic kidney disease are also at highest risk in developing malnutrition due to different dietary restrictions, inflammation, and protein energy wasting. It is said that malnutrition negatively affects both cognitive functions and quality of life. This study aims to explore the association of malnutrition to the perceived quality of life of CKD patients maintained on hemodialysis in the Philippines.

We conducted a cross-sectional study involving 144 CKD subjects undergoing maintenance hemodialysis at the Outpatient Department of Philippine General Hospital and four (4) nearby outpatient hemodialysis centers within the area. The study was conducted from November 2024 to March 2025. Demographic and clinical data were collected from patients and from dialysis records provided that the patient has given an informed consent. Nutritional status was assessed by using the Mini Nutritional Assessment Short Form tool while the quality of life was measured using the validated Filipino version of Kidney Disease Quality of Life- Short Form KDQOL-SF Version 1.3 (KDQOL-SF ver. 1.3). Statistical analysis included ANOVA and chi-square to determine the associations.

Of the 144 subjects, 38.89% were at risk for malnutrition, 10.42% were malnourished, while the rest scored normal in the MNA-SF. KDQOL-SF analysis showed highest scores on symptoms of kidney disease and social support. Significant associations were found between nutritional status and KDQOL components including symptoms of kidney disease (p=0.004), effects of kidney disease (p=0315), bodily pain (p=0.0005), general health (0.0008), emotional well-being (0.0065), social function (p=0.002), energy (p=0.0121), physical health component (p=0.0633), and mental health component (p=0.0196). In these subsets, presence of malnutrition had lower mean scores compared to those with normal nutritional status.

The Relationship of Nutritional Status and Quality of Life among CKD Patients Undergoing Hemodialysis

The presence of malnutrition is significantly associated with some, but not all aspects in the assessment of quality of life. This includes the major subsets like physical component, mental component, symptoms of kidney disease, and effects of kidney disease wherein there was noted poorer quality of life with deteriorating nutritional status. This confers some benefit in addressing malnutrition in Filipino CKD patients on hemodialysis to further improve their quality of life. However, more studies are still needed and a combination of different anthropometric measurements, nutritional scoring, and quality of life assessment tools can be utilized for better correlation.

This abstract was presented in the 45th Annual Meeting of Korean Society of Nephrology (KSN 2025) held at Coex, Seoul last June 19-22, 2025.
