Dyslipidemia in patients with advanced Chronic kidney disease is highly atherogenic and associated with development of atherosclerotic cardiovascular diseases. Though it is frequent in patients on maintenance hemodialysis, there is insufficient data on the impact of hemodialysis on lipid profile. Our aim was to evaluate the lipid profile before and after hemodialysis of patients with kidney failure
We carried hospital based cross sectional analytic study from 1st February to 8th September 2025 at the Yaounde University Teaching Hospital. We included consenting adult patients on maintenance hemodialysis for at least 3months who were fasting for at least 6 hours prior to their HD session. We excluded cachexic patients, those on lipid lowering drugs and all hemolyzed samples. We collected 5 ml of blood in a dry tube before and after hemodialysis, for the measurement of total Cholesterol, HDL Cholesterol and TG. While LDL and Atherogenic index of Plasma were calculated. Cardiovascular risk were classified using Artherogenic index of plasma as low =AIP˂ 0.11; moderate =AIP was between 0.11 and 0.21 and High for AIP ; ≥ 0.21
We included 83 patients (49 males) with a median age [IQR] of 47years [37-56].The median dialysis vintage [IQR] was 12.4[25.6-96.2] months. Hypertension (85.5%) and diabetes (22.9%) were the most frequent comorbidities. The median values of total Cholesterol, HDL and LDL increased significantly after HD (see Table I) and could be attributed to hemoconcentration due to ultrafiltration. However our findings are contrary to those of Siani et al in 2021 in India. Furthermore the median values of Triglyceride and Atherogenic Index of Plasma (AIP) remained constant (see Table I). More than 50% of patients had high cardiovascular risk before and after hemodialysis using AIP (see Table II), hence re-iterating the fact that patients with Kidney failure on Maintenance Hemodialysis have very high Cardiovascular risk
