Chronic kidney disease patients  who  undergo hemodialysis (HD) and Continous Ambulatory Peritoneal Dialysis (CAPD) often experiences disturbance in nutritional status and the quality of life. This  condition is affected by the dialysis modalities and treatment duration. The  previous studies have shown inconsistent results. This study aims to assess the differences in nutritional status and the quality of life in patients  undergoing hemodialysis  with 4-hour, 5-hour  duration, and CAPD in order to determine the best modalities.

This cross-sectional study was conducted on CKD patients who underwent dialysis in several dialysis centers in Malang, East Java, Indonesia (Hemodialysis Clinic and CAPD Clinic in dr. Saiful Anwar General Hospital, Hemodialysis Clinics in Panti Nirmala Hospital, and Hemodialysis Clinics in Wava Husada Hospital).

Participants were divided into three groups: those undergoing CAPD, 4-hour HD, and 5-hour HD. Both hemodialysis groups were treated twice per week. Data collected included demographic and baseline characteristics, nutritional assessment using the Subjective Global Assessment (SGA), and evaluation of quality of life through the 36-Item Short Form Health Survey (SF-36).

Statistical comparisons were made to examine differences in nutritional status and quality of life between groups. The investigation was approved by the ethical committee of the study site, and all procedures were performed in accordance with the Declaration of Helsinki.

In total, there were 377 patients (152 in 4-hour HD, 147 in 5-hour HD, and  78 in  CAPD). Patients in the 5-hour HD group had better results according to several variables compared to the 4-hour hemodialysis and CAPD. They had higher SF-36 physical and mental component scores, and better nutritional status (p < 0.05).

Hemodialysis with a duration of 5 hours provide better nutritional status and quality of life compared to a duration of 4 hours or CAPD.
