Anemia is considered a risk factor for morbidity and mortality in chronic kidney disease. It is characterized by a normocytic normochromic state that presents with decreased erythropoietin activity due to renal impairment. The prevalence of anemia in patients with grade 5 kidney disease on hemodialysis according to the USRDS has the following distribution: 64.5%, 14.4%, and 6.6% based on the respective hemoglobin levels: 10-12 g/dL, 9-9.9 g/dL, and <9 g/dL. Classifying according to KDIGO guidelines the different phenotypes of iron deficiency being these the "systemic iron deficiency" with ferritin <200ug / l and transferrin saturation <20% and "erythropoiesis with iron resistance" defined as ferritin >200ug / l and transferrin saturation <20% in correlation with hepcidin protein that is expressed in the liver mediating the flow of iron in plasma and extracellular fluid. Resistance to erythropoiesis-stimulating agents is characterized by excessive doses to maintain target hemoglobin levels with resistance index defined as weekly dose per kilogram of body weight divided by the hemoglobin level being mainly absolute resistance.

OBJECTIVE To identify the prevalence of anemia and iron deficiency phenotypes in patients with anemia due to chronic kidney disease on renal support therapy with hemodialysis, in association with serum hepcidin levels.

Methods Analytical cross-sectional study. 105 patients on hemodialysis therapy were included, of whom 92 met the inclusion criteria. Clinical characteristics were analyzed with measurement of serum hemoglobin, serum iron, transferrin saturation, and ferritin levels, and determination of serum hepcidin levels.

Ninety-two patients with chronic kidney disease receiving renal support therapy with hemodialysis were identified. 58 were men (63%) and 44 were women (37%). Their ages ranged from 20 to 70 years, with a mean age of 45 years. The mean length of stay on hemodialysis was 5 years, with a range of 1 to 10 years on this renal support therapy. The mean hemoglobin level was 9.81 g/dL, with a range of 6.2 to 15.2 g/dL. The prevalence of anemia was distributed as follows: 51%, 20.7%, and 28.3%, based on the respective hemoglobin levels: 10-12 g/dL, 9-9.9 g/dL, and <9 g/dL. Iron deficiency phenotypes were identified in 34.7% (32) patients with “systemic iron deficiency” and 65.3% (60) patients with “erythropoiesis with iron resistance”, with hepcidin level in patients ranging from 39-2088 ng/ml with an average of 198 ng/ml. 42 patients (46.5%) were identified as being in target according to the hemoglobin level based on the KDIGO guideline and 50 patients were identified as being out of target range with a hemoglobin value <10 g/dl (54.35%).

The presence of anemia associated with chronic kidney disease is a risk factor for morbidity and mortality present in up to 54.35% of patients. It is of utmost importance to identify the iron deficiency phenotypes recently described by KDIGO. These include iron-resistant erythropoiesis, which occurs in 65.3% of patients, and systemic iron deficiency in 34.7% of patients. Serum hepcidin levels average 198 ng/ml. It is noteworthy that the majority of patients have iron resistance when generating erythropoiesis, compared to those presenting systemic iron deficiency in the hemodialysis patient population.
