Individuals with Chronic Kidney Disease (CKD) often present with multiple comorbid conditions that adversely impact their health and quality of life, including poor physical, cardiovascular, and cognitive function, fatigue, and depression. To mitigate these comorbidities, patients are encouraged to increase their physical activity. However, this guidance is often provided by a health care team (nephrologists, nurses, dietitians) with little to no formal training in exercise prescription. In other cases, patients are referred to exercise professionals who, while experts in exercise for the general population, may have a limited understanding of CKD pathophysiology and its complexities. This creates a critical knowledge gap that can hinder safe and effective care.

To address this knowledge gap, the Global Renal Exercise Network (GREX) developed a comprehensive training program consisting of two main parts. The first is a free online curriculum with eight learning modules: (1) CKD Pathophysiology and Treatment, (2) Understanding CKD from the Patient Perspective, (3) Overview of Exercise in CKD/ESKD, (4) Screening, Needs Assessment, and Outcome Testing, (5) Strategies for Exercise Prescription in CKD, (6) Behavior Change Principles/Strategies, (7) Class Management Techniques, and (8) Implementation Strategies. The second part is a hands-on internship program to reinforce these concepts with practical skills training.

The online training modules are now freely available to a global audience on the GREX website: https://grexercise.arizona.edu/exercise-ckd-course. To facilitate practical learning, an international network of internship sites is being established in approximately 10 countries across North America, South America, Europe, and Australia. This network is poised for expansion in the coming years to increase training capacity worldwide.

This program is designed to equip both health care and exercise professionals with the necessary knowledge and skills to prescribe and support physical activity for individuals with CKD. By bridging the educational divide, this initiative aims to ensure that exercise programs are implemented in a safe, effective, and evidence-based manner.
