Zinc is an essential trace element that has multiple vital functions to maintain life. Of note, zinc plays an important role in the regeneration of skin tissues and healing process of wound injuries. However, there has been scant report for these functions of zinc. Zinc deficiency was suspected by three criterion including the related symptoms, no other pathogenic condition which explains these symptoms, and the low serum zinc concentration below 60 μg/dL. It has been known that the prevalence of zinc deficiency is higher in hemodialysis patients than normal subjects. In this background, we investigated the relationship between serum zinc concentration and wound healing effects.

The subjects under maintenance hemodialysis who entered Tokushima University Hospital between April, 2021 and August, 2023 because of skin ulcer or ischemic lesion, or postoperative suture failure were included. The serum zinc concentration was measured on admission and analyzed the relationship between the zinc concentration and prognosis or various biochemical parameters on admission. Since around 80% of serum zinc were bound with albumin or alpha2-macroglobulin, serum Zn concentration should be corrected by the concentration of albumin or protein in order to the serum zinc levels properly. We also adopted the two kinds of albumin- or protein-corrected equation of serum zinc concentration (correction-1; corrected Zn=measured Zn+9.5X(6.5-serum protein concentration)+6, Nutr Res. 2014, Correction 2; corrected Zn=measured Zn-3.25X(serum albumin concentration-4.5)+6, Scand J Clin Lab Invest. 2022)and reevaluated these relationship.

Twenty-nine subjects were recruited and mean value of zinc concentration was 53.2±12.8μg/dL(median value 55)and prevalence of suspected zinc deficiency (Zn<60μg/dL) was 23 subjects out of 29 (79.3%). When Using correction 1 and 2 equation, the prevalence was 7 out 29 (24.1%) and 14 out of 29 (48.2%), respectively. The median values of corrected zinc concentration were 67.6 or 59.9, respectively. Our observation detected 4 deceased subjects. When we compared the zinc concentration between the dead and alive subjects, significant differences were detected only when the equation 1 was adopted (57.2±13.8 vs. 77.0±6.9 p<0.001). Linear regression analysis revealed that hospitalization days were significant correlated with zinc concentration by any corrected equation (uncorrected; r2=0.191, p=0.017, equation I; r2=0.157, p=0.032, equation II; r2=0.169, p=0.0268). Other factors relevant to the hospitalization duration were serum CRP levels (r2=0.238, p=0.0073) and hemoglobin levels (r2=0.371, p=0.0005). The significant correlation was also detected between the zinc concentration and serum CRP levels (r2=0.282, p=0.003) on admission day.

Our study suggested that the prognosis of the hemodialysis subjects with skin lesion was suggested to be determined by the zinc deficiency. In the interpretation of zinc concentration the albumin-based or protein-based correction should be considered.
