Membranous nephropathy (MN) is the leading cause of nephrotic syndrome in adults. B-cell hyperactivity plays a crucial role in the pathogenesis of membranous nephropathy. Anti-CD20 monoclonal antibodies are the first-line treatment option for MN as recommended by the KDIGO guidelines. Zuberitamab, an innovative anti-CD20 monoclonal antibody, has shown intense antibody-dependent cellular cytotoxicity. To date, studies assessing MN patients treated with zuberitamab are scarce. This study aims to evaluate the real-world effectiveness and safety of zuberitamab in MN patients.
This multicenter retrospective study enrolled patients from 11 hospitals in China, who were diagnosed with MN and administered treatment with zuberitamab between June 2023 and June 2024. Comprehensive data were meticulously recorded at baseline, 3 months, and 6 months following treatment, encompassing crucial indicators such as 24-hour urinary protein, estimated glomerular filtration rate (eGFR), serum albumin, serum creatinine, CD19+ B-cell count, and serum anti-phospholipase A2 receptor (anti-PLA2R) antibody levels. The primary outcome was the remission rate including complete remission (CR) and partial remission (PR).
Totally 115 MN patients were enrolled, with a mean age of 50.42±12.95 years. The remission rate was 62.6% at 6 months, including a CR rate of 24.3% and a PR rate of 38.3%. Significant reductions were observed in 24-hour urinary protein (P<0.001 for 3 and 6 months), CD19+ B-cell count (P<0.001 for 3 and 6 months) and anti-PLA2R antibody levels (P=0.004 for 3 months; P=0.001 for 6 months) at both 3 and 6 months. Serum albumin levels significantly increased (P<0.001 for 3 and 6 months), with stable serum creatinine and eGFR levels (P>0.05 for both). Baseline eGFR was significantly higher in patients who achieved CR or PR (100.90 [87.94, 115.62] ml/min/1.73 m2) at 6 months compared with those who did not (88.00 [60.59, 101.57] ml/min/1.73 m2, P=0.002); baseline serum albumin levels were also higher in patients who achieved CR or PR (P =0.026). No serious adverse events were detected.
Zuberitamab demonstrated favorable real-world effectiveness and safety in MN patients, with significantly reduced urinary protein levels, particularly in individuals with normal or mildly impaired renal function.
