The early efficacy of budesonide enteric-coated capsules in treating primary IgA nephropathy remains to be studied, which holds vital guiding significance to optimize treatment.
Patients with primary IgA nephropathy treated with budesonide enteric-coated capsules at Zhongshan Hospital, Fudan University from January 2025 to June 2025 were analyzed. Budesonide enteric-coated capsules were prescribed according to the clinical situation without any interventions. Changes in urinary proteinuria quantitation and renal function were assessed in duration of 24-weeks treatment.
A total of 144 patients with primary IgA nephropathy were included, with a mean age of 43.09±11.56 years, including 80 males (55.56%). Urinary total protein (UTP) was 1.94±1.9 g/d before treatment, urinary red blood cell count was 241.64±69/uL with 13 patients had a history of gross hematuria. The baseline estimated glomerular filtration rate (eGFR) was 56.72±29.65 mL/min/1.73m². Sixty patients had received glucocorticoids concomitantly and six had immunosuppressants other than glucocorticoids. By the 8th week of treatment, UTP decreased by 13.19% compared to baseline (P=0.029). UTP sustained to decrease by 36.29% by the 24th week (P<0.001). As to the renal function, the eGFR increased by 5.23% compared to baseline by the 8th week (P=0.105) and finally increased by 9.79% at 24th week with an upward trend (P=0.171).
Budesonide enteric-coated capsules demonstrated an early significant reduction in proteinuria with mild increased eGFR at the 8th week of treatment in primary IgA nephropathy. The early efficacy remained significant till the 24th weeks, suggesting the effectiveness in short-term treatment.
