ISN SRC programmes are designed to provide training and enhance renal services. The supporting centres (SC) are encouraged to team up with emerging centres (EC) within the same region. CMC Vellore, located in southern India, is the SC, and CIHSR Dimapur, located in northeastern India, is the EC separated by more than 2000 km. Both have graduated through Levels C and B of the ISN SRC programme.
This is a qualitative report of the impact on EC renal services and the advantages of having the SC within the same country. The EC team consists mainly of technicians and nurses, with a few internal medicine physicians, whereas the SC is a tertiary nephrology referral and academic centre.
The visit by the SC team helped the EC staff become trained in best hemodialysis practices and infection control measures in the dialysis unit. They were successful in starting tunnelled catheters, which had a direct impact on the CRBSI rates and the longevity of the catheters. They also had regular classes, both virtually and in-person, on CKD care, follow-ups, and the technical aspects of maintenance hemodialysis, as well as various quality metrics to be followed in a dialysis setup. They also had exposure to peritoneal dialysis exchanges and PD catheter insertions, as access to a hemodialysis facility is often difficult in remote locations. The mutual visits were coordinated easily as there were no travel restrictions or border control. Fund management was also not an issue within the same country; procurement of small essential equipment was done through competitive rates offered to SC.
Overall, since both SC and EC were located within the same country, this led to an enhanced understanding of local needs and lacunae, as well as easy transport and logistical access to both centres. Additionally, better coordination for virtual sessions and consultations was facilitated due to the same time zone.
