Transplantation is the best renal replacement therapy because it has a direct impact on the recipient's survival. Part of the binomial evaluation protocol consists of determining risk characteristics for not concluding kidney transplantation.

A retrospective, descriptive, descriptive, analytical and single-center cohort study was performed, which included patients with chronic kidney disease KDIGO 4 - 5, older than 18 years, who were sent with their respective donor, the binomials sent to kidney transplant protocol at the “Centro Médico Nacional” attended in the period from May 2021 to November 2022 in follow-up until the conclusion or rejection of the protocol. A comparison of the variables of interest was made between patients who completed the protocol and those who did not, using Student's t test for quantitative variables and Fisher's exact test or Pearson's χ2 test for qualitative variables. Subsequently, multivariate analysis of those variables with statistical significance was performed to determine the true risk association to non-completion of kidney transplantation.

The risk factors that showed statistical significance for not completing a transplant were: age of the recipient with OR 1.031, CI 95%, 1.006-1.056, p= 0.014; PRA SA Class I with OR 5.389, CI 95% 2.124-13. 671, p= 0.001; pregnancies (1 - 2) OR 3.106, CI 1.393-6.923, p= 0.006; donor infection OR 9.056, CI 3.671-22.338, p= <0.001; anatomical and/or structural alterations OR 23.234, CI 5.263-102.561, p= 0.001.

There are multiple factors inherent to the donor or recipient that increase the risk of not completing the kidney transplant, highlighting in the recipient: age, highly sensitized patient and factors that increase sensitization; in the case of the donor: anatomical-structural alterations and infectious processes.
