Self-management is crucial for people living with chronic conditions. However, little is known about the perception of self-management support at a population level.

To assess the experience of patients receiving chronic kidney disease (CKD) care in the health authority renal programs in British Columbia (BC), Canada, BC Renal periodically conducts a province-wide patient experience survey that includes a validated patient-reported experience measure (PREM). The PREM specifically evaluates patient perception of self-management support based on the Chronic Care Model.


Results of the 2016 patient experience survey indicated that collaborative patient goal-setting, a key element in self-management, has significant potential to enhance overall patient experience in kidney care.


To inform subsequent action planning, we adopted a mixed-method approach to identify and enable improved goal-setting for adults with CKD from the perspectives of kidney health professionals, patients, and family caregivers living with CKD.

We facilitated a series of focus group with patients, family caregivers and health professionals of various disciplines (53 participants) across all renal programs, as well as an action-planning workshop with key stakeholders (42 participants), including health professionals, patients, family caregivers and community partners in BC. Input from these engagement activities informed a provincial strategy and worksheet (finalized in Fall 2020) for all key stakeholder groups to take tangible actions to improve patient goal-setting across the province. 

Results of the subsequent patient experience survey conducted in early 2022 found that patient goal-setting remains a key area of improvement, though some signs of improvement were observed in most kidney care settings, despite the various challenges of past years. 

Ongoing support in integrating goal-setting conversations into routine practice at the local level, and sharing of lessons learned as a provincial kidney care community will likely yield further progress in improving collaborative patient goal-setting in BC over time.
