AKI affects a substantial proportion of hospitalised patients worldwide and is linked to short-term mortality and long-term progression to CKD. The 2012 KDIGO Clinical Practice Guideline established the first definition and staging system for AKI, based on serum creatinine and urine output, but left the interval between AKI and CKD undefined. Now AKI, AKD, and CKD is reframed as a single continuum rather than separate conditions. This continuum now underpins the KDIGO 2026 AKI/AKD Guideline, which for the first time unifies AKI and AKD in one framework.
