Session Description:
This session will continue to address the delivery of CKM across diverse healthcare settings and will circle back to practical considerations around decision-making and how to deliver comprehensive CKM.
Learning Objectives:
Participants will understand:
i) The management of the metabolic complications of CKD in CKM;
ii) How to transition to and deliver terminal care; and
iii) Understand the full scope of CKM and what is required to deliver it.
