ASSOCIATION OF HIGH PARATHORMONE LEVEL AND HYPERTENSION WITH LEFT VENTRICULAR HYPERTROPHY IN MAINTENANCE HEMODIALYSIS PATIENTS

Introduction:

In maintenance hemodialysis (MHD), LVH is a significant risk factorfor cardiovascular death.  Hypertension is a crucial factor for LVH development in ESRD patients. Parathormone has been identified as an important cardio-toxin which causes deleterious effects on myocardial metabolism and function by intracellular influx of calcium and activates cardiac fibroblasts which in turn causes cardiac fibrosis. So early detection of high parathormone and proper management of both hypertension and high parathormone may reduce the cardiovascular burden in ESRD patients. 

Methods:

This hospital based cross sectional study was conducted at the Department of Nephrology in Dhaka Medical College and Hospital. A total 129 Maintenance Hemodialysis patients were enrolled according to selection criteria. Patients were divided into two groups on the basis of LVH. Following completion of the data collection, data analysis was done using SPSS version 26 

Results:

The mean age of the patients was 43.96 ± 14.82 ranged from 18 – 76 years. Most frequent causes of CKD were glomerulonephritis, diabetes mellitus and hypertension. iPTH was 421.15 ± 204.87 pg/ml in LVH Group and 262.60 ± 117.92 pg/ml in no LVH Group. LVM and LVMI were significantly higher in patients with LVH. Mean LVMI was 306.86 ± 104.73 g/m2 in LVH group and 154.11 ± 46.14 g/m2 in no LVH Group. There was no significant difference in Hb, corrected Ca, Phosphate and Albumin between LVH and no LVH group. Systolic and diastolic blood pressure was significantly higher in LVH patients then no LVH patients.

Conclusions:

In patients on maintenance hemodialysis, there is a significant association of LVH with iPTH and systolic blood pressure. This effect is only seen when PTH is substantially elevated and may then be involved in cardiac pathophysiology.
