PAEDIATRIC SPECIFIC ACUTE AND CHRONIC HAEMODIALYSIS PRESCRIPTION & OBSERVATION DOCUMENTATIONS AND ITS BENEFITS.

Introduction:

Paediatric Haemodialysis involves complex care of both clinical and technical aspects due to small weight, difficulty with vascular access, customisation and management of extracorporeal circuit and HD equipment etc. Apart from these complexities in care, children on HD require multidisciplinary team involvement (Play Therapist, Psychologist, Dieticians, Social worker, School teaching at hospital, Nurse specialist, dialysis staff etc). Hence important to have customised HD prescriptions and observations as per recommended guidelines

Methods:

All aspects listed in recommended Paediatric Nephrology guidelines were incorporated in the development of HD prescription/observations which are not normally included in adult HD documentations. Access: type and cleaning solutions, lock solution use, Heparin; to complete customised dose, extracorporeal circuit volume calculation, option for different priming fluid (i.e. Normal saline, Albumin, Blood), UF calculation and limits, Blood flow rate and pump segment options, Dialysate options & range , monitoring and special instructions related to frequency of vital sign monitoring, Heart rate range, Blood pressure range, bolus volume for hypotension/cramps , blood test, medications/ other infusions, food intake during HD, discharge status, communication with family, Blood Volume Monitoring information etc. Three documents developed were; Acute HD Prescription and Observation, Chronic HD Prescription, Chronic HD observation 

Results:

All documents were evaluated after 6 months of its use and benefits, updated based on feedback.

Conclusions:

These three documents have resulted in fulfilling the complex care needs of Paediatric HD.
