Introduction:
Autosomal dominant polycystic kidney disease (ADPKD) has been described to be associated with multiple extrarenal manifestations such as cerebral aneurysm ,hepatic cyst ,diverticula, pancreatic cysts but rarely any association been described with a giant fibriod. We would like to report a case of such patients diagnosed incidentally.
Methods:
A 30-year-old female presented in the Emergency Room (ER) with complaints of severe headache and blurring of vision and drowsiness, initially she was managed in the ER, later CT angiography of the neck and brain was performed, in which she was detected to have an intracranial aneurysm at the junction of leftA1 and ACOM arteries. She also complained of abdominal distention, pain, and abnormal menstrual cycle.
USG's whole abdomen showed a large heterogeneous abdominopelvic mass displacing the uterus to the left with large polycystic kidneys. MRI of the Abdomen and pelvis confirmed that a mass of size 20.1x20.4x10.4 cm is in continuity with the uterus.
Results:
right pterional craniotomy followed by clipping of aneurysm was performed, the patient was discharged in stable condition, later again she was admitted after 1 month followed by total abdominal hysterectomy.
Conclusions:
ADPKD is a complex disorder involving multiple organs, but the association with fibroid has rarely been reported so far. Females presenting with abdominal pain and menorrhagia and a known case of ADPKD must be investigated for some uterine fibroid and adequate management must be done to alleviate the symptoms.
