ROLE OF THERAPY AND PREDICTORS RETARDING DISEASE PROGRESSION IN BIOPSIED CKD PATIENTS: AN OBSERVATIONAL STUDY

Introduction:

There is a paucity of data on the exact contribution of various diseases leading to Chronic Kidney Disease(CKD) state and its management in advance renal failure state, the therapeutic benefits or harms achieved by giving immunosuppression based on biopsy findings and long-term follow-up data post-biopsy in CKD population. This study aims to determine the role of therapy and the factors retarding CKD progression in biopsied patients.

Methods:

A Retrospective observation study included CKD patients undergoing indication biopsy from Jan 2005 to Jan 2024.

Results:

We included 535 CKD biopsied patients with 78.1% males with a median follow up time of 16 months (IQR 5-51). Post-biopsy bleed occurred in 41 patients (7.7%). Chronic glomerulonephritis (CGN) and Chronic Interstitial Nephritis (CIN) constituted 86.9% (n=465) and 13.1% (n=70) of study population. IgA Nephropathy (n=182; 34%) and CIN of unknown etiology (n=42,7.8%) was the most common CGN and CIN illness. Comprehensive medical therapy (CMT)  including either Angiotensin Convertase Enzyme inhibitors, Sodium Glucose Like Transporter-2 inhibitor ,Mineralocorticoid Receptor Antagonists or their combination was given in 47.4%(n= 254) with immunosuppression added solely or in combination with above in 55.1% (n=295) population. On multivariate analysis, CMT and immunosuppression given were found to retard the progression with OR 6.56 (CI-1.30-32.9); P=0.022 and OR 3.16 (CI- 1.5-6.65); P= 0.002 respectively with minimal rates of infectious(24.1%) and non-infectious(10%) complications.

Conclusions:

With a large number of CKD biopsied patients, this study suggests to assess risk and benefits of doing renal biopsy to offer an appropriate therapy which may retard CKD progression.
